KMG Healthcare is seeking a travel nurse RN Home Health Hospice for a travel nursing job in Napa, California.

Immediate Registered Nurse (RN) - Home Health Hospice travel contract opportunity supporting patients in Napa, CA.

Important Requirements:

\t
• Must have CA license 
\t
• RN needs to have 1+ year Home Health worked in last 3 years
\t
• HCHB charting required

Pay:

\t
• $2,928.00 (Travel pay package)
\t
• Local pay package available
\t
• Mileage reimbursement for patient related travel

Work Schedule:

\t
• 11:00am - 7:30pm (Monday to Friday)
\t
• 40 hours per week
\t
• (MUST be able to drive when dark, able to do day shift / swing, and must commit to at least 2 weekends, preferably 3 per month.)
\t
• Territory: Napa County (city and rural areas)

Contract Details:

• 13 week contract (possible extension)
\t
• Nurses are paid by the hour, not by the visit

Job Details

\t
• Perform comprehensive assessments and develop individualized hospice care plans
\t
• Deliver skilled nursing care, symptom management, and emotional support
\t
• Educate patients and families on hospice philosophy and comfort care practices
\t
• Collaborate with the interdisciplinary team to review and adjust care plans
\t
• Maintain accurate documentation in compliance with hospice standards

Qualifications

\t
• Current RN License
\t
• Must have at least 12 months of home health experience within the past 3 years.
\t
• OASIS experience required
\t
• Hands-on HCHB experience required
\t
• Current driver's license
\t
• American Heart Association BLS
